some more dictionaries:

Chambers 1998 [British]: a dish served at dinner between the chief
courses, ie between fish and roast, also (esp US) a main course and
(esp Austr) a starter

Collins Cobuild 1995 [proposes to be transatlantic]: At restaurants
or formal banquets, the *entrée is the main course, or sometimes a
dish before the main course.

Macquarie [Australian]: a dish served at dinner before the main course

and then, from Jeremy Smith's _Bun Bags and Fanny Packs_,

   in the Brit ---> Am section: entrée  starter, first course
   in the Am ---> Brit section: entrée  main course

(the use of "starter" in a purportedly american gloss is odd.)
