I didn't, and don't; I usually use one "and" (e.g., "one thousand[,] one
hundred and one") in speaking ... although I think it's generally fine and
the same without any "and", or with two "and"s (which I don't encounter much).

I suppose pragmatism would favor "and" in some cases (e.g., "one hundred
[and] five dollar bills", in speech).

Not me; I use "and" even more (e.g., "one thousand and one hundred and one
and 00/100"). Nobody's refused one because of this idiosyncrasy, so far.
