It means "existing before the effective date of the insurance policy." The
standard used to determine if a condition is pre-existing or not is whether
the patient has sought medical consultation for it.

"Pre-existing" doesn't just mean "existing." It means "diagnosed, or having
exhibited signs that a physician should have investigated and then
diagnosed."
