Reminds me of when my brother (now 64 but younger then) used to refer consistently to chicken or veal cutlips.  I guess that would work for doctors too.

Is that still pronounced "extra munction" I wonder?  

Not a doctor's nickname (I suppose "Bones" is one of the less pejorative ones) but a saying that suggests that once upon a time docs were low professionals on the totem pole where lawyers are now:  "Mentiris ut medicus" ('You lie like a doctor').
