Dear Anna,
     I tried basically replicating your problem with INSERT, but
everything works fine for me.  One minor point is that you should
normally use the +c switch to add a corpus name, but otherwise your
file headers look right etc.  The best approach would be to directly
send me a copy of the file as an email attachment and I will try to
replicate it with your file.  You might want to make sure that CHECK
runs okay first.
