We are running into a similar issue in my lab.  We have four precodes:
English [- eng], Spanish [- spa], Mixed [- mix], and unassigned [- una].  I
added the mix and una to the ISO-639 file and the files pass CHECK.  Doing
it this way allows us to quickly sort those utterances that are so
unintelligible that we cannot assign a language, or where what is said
could be either language (both types are coded with the UNA precode), and
those that contain code switches (the MIX precode).  My bet is that later
on we will be going back to the MIXed utterances to further code the nature
of the code switches, as you describe below with @s...

Sorry I can't answer the question about the +l switch.
