lang questionnaires for ages 3 & up

Does anyone have recommendations for language development
questionnaires that are appropriate for screening children who are 3
to 10 years of age.  I am specifically interested in questionnaires
that parents can complete quickly and easily (i.e., something akin to
the short forms of the CDI but for older children).
