Subject: Looking for Bilingual Self-ratings Survey Instruments

Hello, I have a McNair Scholar starting a senior thesis about
second-generation Hmong Americans' self-perceptions of and confidence in their
L2 English skills. She'd like to do an oral survey, and we're looking for
models. Bilingualism/SLA is a bit out of my area -- can anyone point me toward
studies that have used surveys (or other instruments) to gather bilingual
speakers' self-proficiency ratings and especially confidence in various
language-use settings?

Thank you!
