Sullivan and Neville Stiles. Edited by Wick R. Miller and Karen Dakin. Salt Lake City: University of Utah Press 1988. If anyone has a copy they no longer use please let me know. I've looked everywhere for it..all major out-of print bookstores. Even the British Library doesn't have a copy!
