<Way to go!> Explanation

This has nothing to do with Russian (or Slavic), but I hope my North
American colleagues will help me out and others will bear with me. I'm
interested to know 2 things:

1.  What PRECISELY does the expression <Way to go!> mean? It
    seems to me to mean something like <Well done!> <Good for
    you!>, or similar. Examples of its use would be helpful.

2.  What is its derivation/history?
