The paper is not published but can be accessed on the web. Unfortunately,
the address is one of the things that got lost when I moved to Australia.
But you can ask James himself at jmacfarl at unm.edu, I'm sure he'll direct
you to the webpage.
Terry Janzen also told me he had a paper coming out about this topic. You
can ask him at tjanzen at post.rrcc.mb.ca and ask him.
