I think it was The Prisoner episode "The Chimes of Big Ben" where I heard
Fotheringay pronounced as spelled...that sets it back to 1968 or '69...
The one that threw me most recently is Dalziell (from Dalziell and Pascoe,
imported by A&E), which, near as I can tell is dee-ell.
