"... who claims to have coined the expression."

How many of the expressions in this posting are
"coined"?  Does saying something once constitute
coining a phrase?  Does a variation on a common phrase
constitute coining of a new phrase, i. e.,
"pencil-neck geek", when "pencil-neck ...fill in the
blank..." or just plain "pencil-neck" as a noun have
been around for decades?
