Re either/patio/envelope I'm wrestling with (and collecting definitions for) distinctions among labels such as _elevated, refined, educated, fancy, affected_. I seem to have a continuum (though I didn't grow up hearing 'refined') keyed to appropriateness for gender within region: anybody else?
