A Google Groups search for the initialism BSOD turns up a reference to the
"Black Screen O' Death" from 7 September 1993, in conjunction with Windows
3.1 and Novell:

This post in turn references an article by [Robert X.] Cringely in
InfoWorld on 12 April 1993. It might be worth checking out.
In the course of performing these searches, I have run across references to
the initialism BSD, meaning the same thing as BSOD. But the mind reels at
the thought of trying to do a Google search on the term BSD.
