A Question on Questions

It's fairly common, in contemporary English, to express a yes-no
question simply by changing the intonation on the corresponding
declarative, as in, e.g., "You agreed?" Is there anything known about
how long this form has been in use, as opposed to the inverted form
"Did you agree?" (I'm likewise curious about such forms as "You gave it
to who?" or even "You gave it to...?")
Anyone know anything about this?
