I don't think it is a 'real' English word, not even widespread English
slang or jargon. I think it's plain Italian, meaning essentially
"joker", also an adjective ("jocular" or so), not _necessarily_ an
insult. I think it appears rarely in English speech or text, as any
foreign word might: I find only a very few instances in English on-line.
