Yes to both Herb and Charlie. Even worse, the majority of first-year college comp courses are (or used to be) taught by literature students who generally have spotty knowledge of traditional grammar, and know nothing about the psycholinguistics of writing and reading. They mostly have to teach by instinct. In the best programs their supervisors give them instruction in what the teachers should and should not "flag," but there are much more important issues than subtle "awkwardness" in pronoun reference that scarcely create significant intelligibility problems.
