"given name"

[Margaret Lee]

Wouldn't  that be the "birth name," consisting of the first name (given name), any middle 
name(s),  and the last name (surname) bestowed at birth?

I take "given" to mean given to when you are born. That seems to apply in
Meg Ryan's case.
