Well, no.

It actually says:

"Some names, places and incidents
are fictitious
and
have been changed for dramatic effect."

Should the "and" be an "or"? Absolutely.

But, some names, places and incidents are fictitious, and some names,
places and incidents have been changed for dramatic effect.

Just not the same ones.
