"Madonna suffers wardrobe malfunction at Thailand show"

Judging by the picture accompanying the story Madonna was in a costume that included a floor-length wedding veil.  "In video footage filmed during the concert, Madonna is seen getting trapped in the veil as two dancers attempt to pull it from her, and she fights to free herself as she is dragged backwards."

Previously the term "wardrobe malfunction" meant a woman was seen topless due to a problem with her clothing.  Here Madonna apparently remained full clothed (except for the veil) and the malfunction was that she was dragged backwards.
