i think they'd all be understood today, though maybe only "puke" is particuarly common.

"upchuck" and "puke" are about a generation older than "barf" and "ralph" (the relative newness of "barf" and "ralph" might have contributed to their popularity, in certain circles, for a while).  "heave", on the other hand, goes back well into the 19th century (GDoS has an 1862 Artemus Ward cite with the irregular post "hev").
