The Oxford book is a revised version of (parts of) the thesis. The
stuff on coordination didn't make it into the Oxford book. However,
there's now a very much more user-friendly (read: informal) version of
the two relevant thesis chapters that's just been accepted for
publication. If anyone is interested, let me know.
