The International TESOL organization provides a directory of undergraduate,
graduate, certificate, and other programs, at their web site:
http://www.tesol.org/s_tesol/seccss.asp?CID=124&DID=1594
The American Association of Applied Linguistics (AAAL) provides a directory
of graduate programs of institutions that are members of AAAL:
http://www.aaal.org/graduatePrograms/
Neither of these directories provides rankings, of course.
One reason that you don't see the Ph.D. in TESOL at many institutions is
that the MA in TESOL -- a Master's in Teaching -- is often considered a
terminal degree.
