I began by googling
	"Canadian American Slavic Studies published by"

which gave me the publisher's name: "Charles Schlacks, Jr."

Then I googled
	"Canadian American Slavic Studies" "Charles Schlacks, Jr."
and got <http://www.schlacks.com/journals.html>, which is the 
publisher's website.

Click "Contact us" and you'll see an email, a phone number, and a post 
office box.
