MIM: deaf/Deaf-distinction: origin?

Tom,
Many thanks for your e-mail and I do very much
appreciate your correction here.
As the distinction had been made due to the discovery
in Woodwards (1972) sociolinguistic works placing deaf
people into two groups based on the language, it must
be very difficult to track down to how this leads to
the labels of d and D based on two groups of deaf
people....?
Perhaps anyone could shed a light on this, I'd be
extremely interested to know the outcome...
