I'd go for the upper left one.  That feels normal.

which one would you choose?

When we are using the numbers as page numbers (very tiny at the 
bottom of each page), is the Rotation Arrow really necessary?

Is it ever necessary in this case, since we are writing both hand 
positions anyway?
