To answer your question, from the inside of the palm is more common. I know,
it doesn't follow general ASL rules, but I think that this became an
exception because the cake come from the inside of a pan, not the over
turned underside of the pan. If that doesn't make sense, tell me or just
ignore it. ;-)
I usually write Ice Cream with the half black and white square. And are you
scooping the Ice Cream outside of the bowl? ;-)
