Updated ISWA guide, now with UTF-8

Hi List,

I just finished version 2 of the ISWA 2008 HTML Reference.  This is a 
static guide that require neither the internet nor a personal web server 
to use.

This update adds UTF-8 characters for every symbol in the ISWA 2008.

This update also corrects the name of 2 BaseSymbols. 
Mistake:
BaseSymbol 135...01-06-002-02...IndexMiddleBaby on Hinge (wrong name)
BaseSymbol 136...01-06-002-03...IndexMiddleBaby on Angle (wrong name)

Should be:
BaseSymbol 135...01-06-002-02...IndexMiddleRing on Hinge (correct name)
BaseSymbol 136...01-06-002-03...IndexMiddleRing on Angle (correct name)
