I think this may also be a move towards increasing acceptance of
alternative forms of medicine. More insurance plans cover things like
accupuncture, chiropractic, etc. so a word like "physician" would
exclude most of those care providers, and even "doctor" would exclude
some (chiropractors are usually a kind of doctor but accupuncturists
usually are not - and when you say "doctor" most people probably think
you mean "MD".)
