Here are two well-designed questionnaires validated for research purposes
with bilingual adults:

1. Language Experience and Proficiency Questionnaire (LEAP-Q; Marian,
Blumenfeld, & Kaushanskaya, Journal of Speech, Language, and Hearing
Research 2007, 50, 940-967; also available from v-marian at northwestern.edu ).



2. L2 Language History Questionnaire (Li, Sepanski, & Zhao, 2006; also
available at http://cogsci.richmond.edu/LHQ.php ).
