Not to mention Frederick, Ivan (Ivan III, that is, not his grandson Ivan IV
"the Terrible"), Leo, Alfred, Otto, Akbar...

I believe it usually means that the person had a significant impact on
history, at least within his or her own country.

In Peter's case, it could also be taken as a literal reference to his
physical size - he was 6 feet, 7 inches tall.
