It is not unheard of for a language to have different dialects in 
different countries: Spanish in Argentina vs. Spanish in Colombia.

The same may be true between Russia and Ukraine. At this point we can 
say with certainty that "v Ukraine" is said in Ukraine, "na Ukraine" 
is said in Russia. However, both dialects more and more often say "iz 
Ukrainy".
