SL-to-SL interpreters

Since this came up in our discussion of the Amsterdam Manifesto, I
was hoping someone could let us know what the current situation is. At
TISLR 6 I was dismayed (but not really surprised) to see that talks
delivered in a signed language were typically going through two
interpreters to get to Deaf attendees who didn't know that language. Are
there many SL-to-SL interpreters? Which countries have more of them?
