The OED mentions the abbreviation "U.S.A." in its entry for "U.S.," but does not present any citations.  The earliest citation I can find in some quick research is the following:

1776 _Pennsylvania Evening Post_ 3 Sept. 437 (America's Historical Newspapers)  Resolved, ... That the Inspector mark each cask approved with the letters U. S. A.
