May I also add that you can find such a questionnaire, and extensive
discussion of the role of the input in bilingual development, in my
book:

You may also be interested in the questionnaire I used for a survey of
language use in about 18,000 families, including immigrant families.
You can find this questionnaire in:
De Houwer, Annick, 2007. Parental language input patterns and
children's bilingual use. Applied Psycholinguistics, 28:3, 411-424.
Of course, as Dr. Baker hinted at, there is always a need to adapt
existing questionnaires to specific research questions and
circumstances.
