I think Kim Blewett had a good overview of suggestions what to do.

Another possibility for the clipped top of the headwords -- maybe the
header is too high on the page. Some printers don't print close to the edge
of the paper. If the print preview in Word showed the charaters without
being clipped but the printout has them clipped, this is what I would
expect.  You could try increasing the number in "From edge, header" in the
Word margins dialog. You should be able to get to margins by going to
Format, then Document.  (this is in Word 2004 for the Mac).
