Sorry, I also could have mentioned that chapter 3 of my thesis contains a long discussion of the development, via grammaticalisation, of a new auxiliary verb in Mudburra (Ngumpin, Paman-Nyungan). The thesis is available via the University of Queensland's eSpace.

I thought you might find the case of hacer  (do) bilingual verb constructions in Spanish–English code-switching of interest. Below, I’ve included a couple of relevant references, as well as a recent abstract by Iarina Albu (MA student at Utrecht University), co-authored with Roberta D’Alessandro (both in cc). Osmer Balam (also in cc) has done extensive work on this.

Perhaps the many examples of "ordinary" and "replica" grammaticalisation in Heine & Kuteva (2003) might be what you're looking for? (Though they focus on grammaticalisation in contact situations, not solely in code-switching, as I recall.)

I am wondering if anyone has reported any case of grammaticalisation emerged in code-switching.
The grammaticalised construction is not a replication of an existing construction in the code-switching languages because these languages do not have similar constructions.
