бы before or after verb / быт ь/ complex sentences

But бы cannot be placed before быть, I suppose:

Он был бы рад, в огонь и в дым,
за вас погибнуть дважды...

What I am not sure of is whether the order is possibly optional in the 
second part of a complex sentence with two "бы" s?
Если бы она не вышда за него, она вышла бы за другого.
or
Если бы она не вышда за него, она бы вышла за другого.

I was taught to place it before the verb. Я бы хотела... I believe the
relevant examples are in Golosa Book 2.

PS: is there a second edition of Golosa book 2 out there? I knew there was a
second edition for the first one.
